Becoming a Patient

Step 1: Please Pre-pay for your consultation by choosing an appointment to speak with one of the Wellness Care Coordinators or the Doctor on duty nurse, who will walk you through the process and make sure you are prepared to receive all the necessary on-boarding and documentation forms.

Step 2: You will receive an email (Email #1) that will contain the initial New Patient Welcome forms that will need to be completed. Once ALL the forms in this email are electronically completed, hit the “Submit” button to send them off to us.

Step 3: Once Step 2 is complete, you will receive a second email (Email #2) which will have several medical records release forms. Complete as many of these forms as you need so we can send them off to your individual physicians/ practitioners so we can better understand your qualifying medical conditions. **Please complete one form for each provider.

Step 4: After we receive your medical records request forms, you will receive a third email (Email #3) which is a comprehensive electronic Patient Health questionnaire. Completing this questionnaire in its entirety is very important as it allows us to treat you and make recommendations based on all your other co-morbid conditions.

Step 5: As we receive records from your physicians/ practitioners, the Doctor on duty will review them and once they are sufficient, he will give his team the permission to schedule you for your actual Medical Visit/ consultation with him that you had already paid for in Step 1. **This step could take days to weeks depending on whether your providers are responsive to the medical records request.

Step 6: You will then meet with the Doctor on duty in person and he will complete a thorough history and physical exam and discuss any questions he may have had on details in your medical records. He will also discuss the benefits, risks, and any evidence-based scientific data of using this herbal plant as a treatment for your condition(s).

Step 7: Should you be deemed a candidate to use medical marijuana as a treatment for your condition(s), you will receive a fourth email (Email #4) that will notify you of such. In that email will also be a copy of the Medical Marijuana Recommendation Form which is a form that has been provided by the State of Louisiana and will be completed/ signed by the Doctor on duty. You can download and print that copy for your records. Our office will fax a copy of this recommendation to the dispensary of your choice so you can proceed to the dispensary to pick up your medication.

Step 8: If you are not a candidate to use medical marijuana as a treatment option, the Doctor on duty will inform you of this and in some cases, request additional records to better understand your qualifying condition.
